By filling out and signing the Player Registration Form and Medical Release Form and a check made payable to "North Vallejo Little League" for $100.00 per child and mail to: North Vallejo Little League ATT: PLAYER REGISTRATION PO BOX 6433 Vallejo CA. 94591 3.) Online below.
I hereby forever release and discharge North Vallejo Little League Inc., and their respective agents, affiliates, officers, directors, managers, employees, members, partners, shareholders and representatives from and against any and all liabilities, claims, demands or causes of action for any injury, illness, incident or damage (physical or otherwise) suffered by my child arising out of, or related to, my child's participation in the North Vallejo Little League (the "NVLL"), including, without limitation, my child's travel to and from sanctioned games.
I hereby represent, warrant and covenant that I have no knowledge of any physical or mental impairment that would affect my child's participation in the league.
Further, I hereby authorize the NVLL staff to act in accordance with their best judgment in any emergency requiring medical attention.
